De-funding Planned Parenthood

"Reproductive health groups say they’re concerned about the Trump administration’s approach to family planning funds, which prioritizes groups that favor abstinence, and its embrace of ideology over evidence-based policies.

The HHS document causing concern among doctors and reproductive health groups mentions “natural family planning” like the rhythm method and other tactics for preventing pregnancy that do not involve contraceptives, according to the Associated Press, as well as other forms of “natural family planning” that don’t include contraceptives. A Planned Parenthood spokesperson told AP that it appears as if the Trump administration is trying to lessen its role in the program through this document emphasizing family planning without contraceptives.

The Trump administration and Republicans in Congress have been focused on defunding Planned Parenthood and appointing people to roles in the Department of Health and Human Services who don’t believe in birth control or abortion access."

********** What makes my blood boil, is that I wonder how many women Trump screwed in his life ..... women who he probably insisted use birth control. And I even wonder if he paid for an abortion or two.

Now he stand there all pious looking like he's been this big pro-birth advocate all his life, pandering to his nutty base. It's sickening. And I'll bet any money Pence has had a hand in this.

Who are the most devoted Catholics, and so most likely to use that method? Priests and Nuns. Look at their birth rate, extraordinarily low.

The 2017 guidelines that I found also included "natural family planning" (this was issued under Obama) because the statute that forms the basis for the grants include that language. So did the 2016 guidelines.

Various news articles say the new doc includes "natural family planning" while articles written a day later tend to add that "natural family planning" is mentioned "several times". The law's not that recent.

I'll wait to see the document in question.

For a few eerie snippets, from "Key Issues" in the funding proposal outline and indications for successful funding applications:

"A meaningful emphasis on education and counseling that communicates the social science research and practical application of topics related to healthy relationships, to committed, safe, stable, healthy marriages, and the benefits of avoiding sexual risk or
returning to a sexually risk-free status, especially (but not only) when communicating with adolescents"

"Activities for adolescents that do not normalize sexual risk behaviors, but instead clearly communicate the research informed benefits of delaying sex or returning to a sexually risk-free status."

Emphasis mine.

The definition of "core planning services":

"Family planning services which offer a broad range of acceptable and effective family planning methods and services (including natural family planning methods - also called fertility awareness), and which includes pregnancy testing and counseling, as indicated. The broad range of services does not include abortion as a method of family planning."

Further definition:

"Family planning services include services for both men and women as they plan their families. Such services include postponing, preventing, achieving, or facilitating the spacing of pregnancy. Family planning services also include infertility services, health screenings and exams, lab tests and related health services which are important to family planning and male and female reproductive health, improving the likelihood of a healthy pregnancy and birth, should conception occur. Family planning services also include information, education, and counseling related to family planning; and referral services as indicated."

Another part:

"Section 1001 of the Act, as amended, authorizes grants “to assist in the establishment and operation of voluntary family planning projects which shall offer a broad range of acceptable and effective family planning methods and services (including natural family planning methods, infertility services, and services for adolescents.)” (Natural family planning methods are also now known as Fertility Awareness Based Methods.) We encourage applications for all methods, especially those that add to the breadth and variety of methods offered in a service area, in particular those with a holistic vision of health and those historically underrepresented in the Title X program."

Under the FAQs, where grantees are asked if they must provide birth control, the answer refers back to "natural family planning", and talks about "risk-free" some more"

Q.) Do contraceptive methods and services need to be provided by the grant recipient? Is contraception considered a part of “broad-based” family planning services?

Yes, Title X regulations require that projects must “provide a broad range of acceptable and effective medically approved family planning methods (including natural family planning methods) and services (including infertility services and services for adolescents).” (42 CFR § 59.5(a)(1)) This requirement is based on the statutory requirement that “voluntary family planning projects . . . shall offer a broad range of acceptable and effective family planning methods and services (including natural family planning methods, infertility services, and services for adolescents).” 42 U.S.C. § 300(a).

Yes, this Funding Opportunity Announcement requires Title X projects to offer a broad range of voluntary family planning methods and services, including information, and education related to family planning, preconception care, contraception, natural family planning, and infertility services. Such methods and services range along a continuum from (1) infertility services to (2) natural family planning and contraception to (3) sterilization, tailored to the unique needs of the individual. This includes all required services as stipulated in 42 CFR § 59.5, which ensure breadth and variety among family planning methods offered. The broad range of services does not include abortion as a method of family planning. Single providers who have developed expertise in one family planning approach or method may be partners in a broader proposal so long as the total proposal offers a broad range of family planning methods.

Q) What is meant by “returning to a sexually risk-free status?”

If individuals who are currently choosing to engage in sexual risk behavior make a voluntary choice to avoid future activities that place them at sexual risk, their health prospects are improved and negative risks associated with personal sexual choices are avoided. The FOA identifies key issues that should be considered in developing the project plan. One key issue is placing a meaningful emphasis on education and counseling that communicates the social science research and practical application of the benefits of avoiding sexual risk or returning to a sexually risk-free status in order to promote optimal health outcomes for all Title X clients.

Q) Is there a specific focus on programs that encourage sexual delay?

Sexual delay and the related reduction in the number of lifetime partners are protective factors for an individual’s sexual health. Including this focus in the funding announcement places a meaningful emphasis on the holistic and optimal health outcomes of every client.

Consistent with the Title X regulations and the intent of the statute, projects must “provide a broad range of acceptable and effective medically approved family planning methods (including natural family planning methods) and services (including infertility services and services for adolescents).” (42 CFR § 59.5(a)(1)).

The Key Issues listed in the Funding Opportunity Announcement (see FOA, page 11) includes an emphasis on multiple science-informed topics, including sexual delay, which should be addressed as part of the Title X family planning service project. Educational services for adolescents should not normalize sexual risk behaviors, but instead clearly communicate the benefits of delaying sex or returning to a sexually risk-free status.

Essentially, they are repurposing funding originally to provide factual, non-shaming sex education, effective reversible contraception, and STD prevention to treatment of infertility itself, required promotion of the rhythm method, and essentially saying that people can apply it they refer people for hormonal contraceptives just so long as they give a referral.
