The amount of flying and stress he's been under, there's all sorts of issues that can crop up with someone of his age and physical condition. He probably binge snacks, as well, and his diet is atrocious.
He has not been looking physically competent at all since he took office.
Another concern would be if he took any food or drink while meeting with Vlad. There's all sorts of delayed action substances that a stubborn toddler type personality wouldn't allow a doctor to check for before it's too late.
