I can't imagine that phone call. I feel it for the family.

Always keep your head down when approaching or leaving the helicopter.
Always stay within the pilot’s field of view and maintain eye contact.
Never move towards the tail rotor or any exhaust.
Never touch the helicopter or sling cable before it has landed or touched the ground.
Always wear your seat belt.
Always latch the seat belt AFTER you get up to leave the aircraft.
If the doors are on, make sure they’re fully closed and latched.
Never exit the aircraft and walk uphill away from it!
If the terrain is uneven, the pilot may indicate that you stay close to the fuselage and remain there as he lifts off.
If you’re not getting on, getting off or loading/unloading the aircraft, you have no business being close to it.
Stay well outside the rotor area whenever the helicopter is landing or taking off.
Use eye protection - always.

I read of an incident a few years ago where the pilot was walking away from a small helicopter and a gust of wind tipped it in his direction. That's all it took.
