The example of his firing people without bothering to inform his staff has to do with his inability to even think about what other people need to know. People don't exist for him unless he needs something from them at the moment, otherwise he literally does not think about them.
Except for those who piss him off/threaten his self image, then he focuses on destroying them.
That's what all the hostile tweets are about, correcting, in his mind, a reality problem.
