There are a couple of other spots on the arm that are good spots for a vein puncture.

and has a history of falls, including one that fractured his shoulder last year. Blood thinners are more common than they used to be. The bandaids all over his hands are likely for abrasions as it's easier to run an IV further up the arm in most cases.

My bet is the guy took a tumble recently and this is the result.
