(52,387 posts)Every transition.
Passwords and combinations are also routinely changed, probably at least once a month for anything classified.
And when anyone loses legal access to it, such an employee quitting or even getting transferred or dropped from a project.
Passwords get changed a lot in secure environments as a matter of course.

(7,638 posts)I work for the DOE and we have two-way cyphers. Meaning, you have a code but also a device that you use to produce another code when login in. Unless you have that little device in your possession and the other pin, you cant log in. Its also possible for the administrators to disable/deauthenticate these devices remotely which is done every time a person leave or if you misplace your device (some are in the forms of USB sticks, others are small devices flashing a number when you click them, etc..)

(25,914 posts)The last time I had a job where I was signing in to the computer every day, my passwords were made invalid even before I'd completed my last shift.
Which was HUGELY frustrating, as all of a sudden I could not do my job. The second time it happened I got ahold of a supervisor and explained that if it wasn't fixed more or less immediately, I'd just close up and go home.
