I thought it was always the Chief Justice.

Maybe it is because it is the Chief Justice who oversees the Impeachment Trial in the Senate?

I didn't read that right, I thought they both were sworn in at the same time by the same person. But that makes sense now that I re-read the OP.

The Bible is used by tradition, but it's really the preference of whoever is taking the oath whether they want to use a book and if so which one. Representative Keith Ellison borrowed a Quran once owned by Thomas Jefferson for his oath of office, and John Adams used a law book. I'm not aware of any incidents where no book has been used, but it wouldn't surprise me if it's been done that way.
