A medical person suggested no lamination because the plastic film can lift the ink and mess up your card.

On one of the copies, in the blank lines added my pneumonia and shingles vax, and laminated the copy for the wallet and another one for a lanyard. If covid updates are done, it will be on the original.

There are extra fields on it for subsequent booster shots. Put it into a plastic protector.

Treat it like a passport, because it very well may become one.

... with the CDC's V-safe on the day of the 1st dose.

-----
V-safe is a smartphone-based tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccine. Through v-safe, you can quickly tell CDC if you have any side effects after getting a COVID-19 vaccine. Depending on your answers to the web surveys, someone from CDC may call to check on you and get more information. V-safe will also remind you to get your second COVID-19 vaccine dose if you need one.

Does it have your pic, DOB, etc on it? Hell, people forge passports, drivers licenses, etc and the have pictures on them. I could put my best friend’s vax record on my phone, doesn’t make me vaccinated.
