…so I’m hoping someone will post it for me, since I couldn’t quite figure it out.

Trudeau is essentially saying Widdle Pucker is doing drugs (“self-vaccinating” in the men’s room). In all my decades of following Doonesbury, he has never made a statement that harsh that could not be backed up with fact. He’s satirical and razor-sharp, but never a liar.

My take was that Tucker, despite his rabid anti-vaccine stance, has been secretly vaccinated. "Self-vaccinated," in this case, a euphemism for how surreptitious Tucker has been -- hiding it from anyone who could call him out. And yes, it has connotations of illicit drug use, but that's for comedic effect.
