Advent Health Consent Form. Has anyone analyzed that crazy form?

There's a pre-check form that you have to sign into and provide consent in order to move forward. I read it through and felt like they really leave you with no option. By the time we see that form the insurance has been picked and we're stuck on the program for a year. No where to go until next year when we change our insurance.

To me, it feels like that form tries to break Hipaa Rules. Crazy things, like they have the right to share our medical records with clergy and business partners. No way to opt out, though I'll do the ask when I go to the visit.

This is how it usually happens with me. In the nineties we went with an HMO which pulled us away from the doctor I liked and in the hands of a quack. And now, I found a doctor I liked and just a few years later, their practice when to Advent Health.

Crazy, crazy, crazy.

I'll go over the consent form and highlight the areas that I found concerning and ask them if they're forcing us to sign a consent form that breaks HIPAA.

This is because of the knock-down drag-out battles the RWNJs have waged to ensure that nothing to do with women’s sex-lives is covered — those are the “conscience clauses” that allow doctors, nurses, and even pharmacists to refuse to sell you contraceptives or Plan B or Morning After pills.

Don’t worry, though — Viagara’s still on.

You asked what possible justification, and I had that thought.
I don't agree, but perhaps they want to be able to inform clergy so they can counsel and offer prayers and support. There were some good suggestions on this thread about how to respond.
I hope the op can remove the offending clauses before signing.

I have them and they are skimming off medical assistance by being a "provider". They tried to wheedle out paying for my transgender treatment.
BuT I called medical assistance directly and they snapped the fucking bigot mormons in line.

Religious organizations DO NOT belong in anything related to healthcare. Keep your bullshit in your church stay in your lane fuckers.
