If you look up the origin of that term, it's from the late 1800's, early 1900's, aka the eugenics era in America from which the Nazi's learned so much from. And it specifically referred to people who breathed through their mouths rather than their nose due to medical conditions, but also ones that often coincided with intellectual or developmental disabilities. Or just variations of humanity such as Down Syndrome. You may as well normalize the so-called, "R-word". And yes, it is still very much meant to imply someone is "stupid or intellectually inferior"; it's never a choice unlike behaviors such as being a Republican, terrorizing people or firebombing donut shops.
