If they don't use the word burger it's hard not to think it means dollars. I wonder if McDonalds somewhere releases the number of burgers or people they serve.
Also, the OP said $21 billion in the title.

checked and couldn't find profit figures, just worldwide revenue- > 28 billion.
I guess they weren't being literal.
