Especially the makeup.
Next I saw him as Caligula. A stunning performance. I'm sure Hurt was a wonderful person, but he seemed to have this knack for portraying evil, insane, or grievously damaged personalities.
Then he popped up in "Alien", where he seemed normal enough until an alien larva popped out if his rib cage.
