You forgot 1.b. Win two-thirds of the Senate.
And, as soon as Trump is impeached, Pence is president and names a new VP.

4. the new veep would need to be confirmed by the senate.

if this is the plan, and we can somehow (fantasy land) muster 2/3rd support for it in the senate, then the senate could easily simply refuse to confirm the veep appointee.

as a technical matter, pence could be removed first, or both could be reverse minutes apart.

in any event, it's all complete fantasy. ain't gonna happen.
