The IG warned that the DOD was not reporting cases to the FBI gun database beginning 20yrs ago.

And when they did it was only if you got a dishonorable discharge.

I could see some nutjobs like "General" Kelly of White House infamy refusing to report cases to the FBI database because they believe everyone has the God-given right to keep and bear arms no matter what. ??? (And maybe they own stocks in gun manufacturers???) Or something. ??? In other words: deliberate or bureaucratic oversight?
