California declares a state of emergency over monkeypox outbreak, following New York and Illinois

California Gov. Gavin Newsom declared a state of emergency over the rapidly spreading monkeypox outbreak on Monday, the third U.S. state to do so in a matter of days. Newsom said the emergency declaration would help support the state’s vaccination efforts. Demand for the vaccines has outstripped supply as infections rise.

Staff at sexual health clinics and other sites have struggled to keep up with the influx of people seeking the shots. California is mobilizing personnel from its Emergency Medical Services to help administer the vaccines. Newsom said the state is working across all levels of government to slow the spread through testing, contract tracing and community outreach.

California’s declaration comes after Illinois declared a public health emergency earlier Monday. New York declared a state disaster emergency in response to the outbreak late Friday. The U.S. has reported nearly 6,000 cases of monkeypox across across 48 states, Washington D.C., and Puerto Rico, according to the Centers for Disease Control and Prevention. The outbreak has spread swiftly since health authorities in Boston confirmed the first U.S. case in May.

California, Illinois and New York – home to the nation’s three largest cities – have reported 47% of all confirmed monkeypox infections in the U.S. New York is the epicenter of the outbreak in the U.S., with nearly 1,400 confirmed cases as of Monday. The Biden administration is weighing whether to declare the a public health emergency in the U.S, according to senior federal health officials. This would help mobilize resources for state health officials that are battling the outbreak. The last time the U.S. declared a public health emergency was in response to Covid-19 in January 2020.

which means an infected person can go 6-21 days before developing symptoms, possibly spreading it by respiratory droplets during close contact the whole time. Cases can range from a flu like illness but with swollen lymph nodes and only a few bumps to a serious illness with high fever and lesions all over the body. It leaves scars but no one yet knows if the scars are permanent. So far, there has been one fatality outside Africa and five in Africa.

We got rid of smallpox, but it looks like orthopox viruses are on the move again. Current thinking is that person to person transmission started back in 2017. Before that, transmission was from infected bats and bush meat and was very rare.

It's still a bit unclear, but for most "maybe" means "oh, yeah!".

Consider

Monkeypox can be spread from the time symptoms start until the rash has healed, all scabs have fallen off, and a fresh layer of skin has formed. The illness typically lasts 2-4 weeks.

Scientists are still researching:
If the virus can be spread when someone has no symptoms
How often monkeypox is spread through respiratory secretions, or when a person with monkeypox symptoms might be more likely to spread the virus through respiratory secretions.
Whether monkeypox can be spread through semen, vaginal fluids, urine, or feces.

Local diseases med school faculty had this whole immensely dissatisfying exchange with the local talk-show host about whether this could even remotely be a STD (answer: "remotely, yes; by most definitions, no&quot said that this orthopoxvirus is present in many of the substances produced by a a body (such as "seminal fluid"--her term), and that the standard accounts say "bodily fluids", and deduced that some patients with primarily initial pharyngeal symptoms acquired it, ahem, that way.
