The first strikes would almost certainly begin as attempts at asserting regional control, perhaps striking critical troop or strategic positions with small scale nukes (while simultaneously redeploying troops to prevent the enemy options of. This sort of thing is bound to ramp up, but it won't start with a doomsday 'preventive' strike).

because as soon as even a small nuke is detonated... both sides have incentive to "fly all birds" (launch everything in the arsenal). The reason is that it is assumed that the other side will try to take out all of your nukes while they are still sitting on the tarmac or inside the missile silos.

It will not be "they detonate a small tactical nuke and take out a US airbase" and we respond with a small tactical nuke and take out a tank column.

This doctrine is the foundation of MAD (mutually assured destruction). Hence the "destruction" in the name.
