We had something like this happen with a horse and a gate. The gate was hung improperly with both hinge pins pointing up.

One of our yearlings stuck his head through the gate, picked his head up, and pulled the gate off the hinge pins. With the gate - fortunately just a four foot wide one - around his neck, he kind of freaked out, running around the paddock and barn yard. His buddies took the opportunity to run out of the paddock and play panicked along with their buddy.

After a couple of minutes trying to catch the original colt, we stood back and let him wear himself out. Once we caught him and lifted the gate off his neck, he was perfectly happy to quietly go back into his paddock and his friends followed.

After that we knew that you screw the top hinge pin in pointing down and slide the hinge up onto it before tightening the hinge.
