A question about a person reporting COVID

If someone had their second Moderna shot six months ago but now tests positive for COVID,
I assume the state health department notifies people with whom he/she has had recent contact.

Is the infected person mentioned by name in alerting others or does the state just say
“You have been in contact with someone” ?

I live in Illinois and they told us they wouldn't use my wife or my names in the notifications - Wouldn't have bothered me TBH, but I guess they err on the side of privacy

On the other hand, I work for the DOD (on federal property), and they didn't conceal my ID.

Although, to be fair when they come and sanitize your cublicle, it's a bit of a clue to your coworkers.
