When I've been to doctors' offices recently, I have complained to the staff about the occasional one or two anti-maskers in waiting rooms. The staff discretely tells them that this is the rule, and if they need a disposable one, they're available at the check-in counter.

my rheumatologist office a couple of weeks ago and a couple of people came in without masks and the front desk staff told them they needed to put on a mask and we’re offered one. They didn’t complain. I get back to the exam room and my dr. walks in with no mask! To be honest I took mine off because my glasses were fogging up. I would not normally do that, but I trust her not to put me in danger. I have been going to her since 1989 and trust her. I did put it back on before before I left the exam room though.
