Nothing could circumvent any ISP from blocking a list or range of IP addresses.
My understanding is that the registrar publishes the name <=> IP relationship in a DNS lookup.
Tier 1 networks are reachable from everywhere but routed access permits IP based rules such as blocking by address.

Sort of. They run the WHOIS and NS records. Name -> IP is run by the NS designee. IP -> name is run by the bloc owner. I run my own name to IP but not IP to name, because it's cheaper that way, but I could run my PTRs if I really wanted to.
