If I were to venture to guess, I'd say there is probably a large rectangular soft box placed high above on camera left and some sort of frontal soft ambient lighting source in line with the camera used as a fill. So there's probably just two light sources for the entire shoot. If I were to do something like this, I'd probably use the video function of a digital SLR while the model moved artistically with the shutter speed set very low, and then merge the images in photoshop.
