Reason I ask, last week I became aware of a MAGAt couple who voted by mail, the husband said this was
the first time they used mail-in voting because of his wife's Dementia. From what I observed she is way
out in left field and can not carry on any kind of conversation. This be voter fraud?

by taking it to the elections board (or administrator) in your county. (often the County Clerk, Registrar, or staff)

Short answer -
Between you and me - this probably is voting fraud - but the only way you're going to get there is getting some kind of outside declaration that this person is incapable and incompetent. (and, really, who is going to want to be pursuing that case? good luck there.) The sticky part is - it is not technically illegal for someone with a disability to receive assistance in physically filling out a ballot - as long as that person is consenting and in control of the process. Nobody wants to disenfranchise these 'other abled' voters (blind, physically handicapped, etc.) - and that's exactly what you would be running into. Can you flatly prove - that this person didn't even know they were voting? And then if you could - can you see any DA or AG in your part of the world that would be willing to take this up?

Sorry - but you'd be far better off sitting on your hands for this one. I suppose you could put it to the husband directly if you really wanted to. But, there again - probably better off not.

Being in an almost solid red town who's hatred of Democrats is in plain sight,
probably best I keep my mouth shut.

Did a YouTube search. It sends me to King5's channel but the video is no longer available.
