4. What's "underwater" mean?

Can anyone explain what this means:
His approval now sits at 38 percent, compared with 58 percent disapproval. ....The Real Clear Politics average of polls shows that Trump's rating is underwater by nearly 10 points.

Isn't that 20%? What does it mean to be underwater at 38% to 58%?
