13. Brandon Lee died by a blank. I think the wad is what killed him.

I find it odd that any live, full blown ammunition was even on a movie set location.

The only thing that I can see happened is someone was shooting the weapon with regular rounds, like at a gun range and it did not get emptied. But that means it is did not checked by someone responsible for the weapons on the set and Baldwin did not check it.

Then again I doubt it is the actor's job to do that.
