it's concern that she's medically unfit. If she weren't exhibiting obvious symptoms of cognitive decline then we wouldn't be having this discussion (for anyone asking "what obvious symptoms?" see for instance her asking Jack Dorsey the exact same question, two times in a row, totally oblivious to the fact that she'd done so).
