Any explanation for what happened must take into account the entirety of the circumstances: Real bullets hit real people - one of whom died, and two more of whom were critically injured. Whether it was a bullet that grazed him or a fragment of something kicked up by a bullet, Trump was hit with something as a result of someone firing bullets at him. It could have been a bullet - bullets are not an all-or-nothing proposition. A bullet doesn't just obliterate everything it hits - it has to hit something of sufficient mass to cause massive damage. If it just grazed the tip of his ear, it could well have done the damage described by Ronny Jackson. But similar damage could have been caused by a more direct hit from something kicked up when the bullet hit something else nearby.
He may well have been exaggerating the severity of his wound, with the cooperation of "his" physician Ronny Jackson. But there was a wound. And any tearing wound would have caused the bleeding we all witnessed.
