You apply for it just like a passport and you pay an extra fee for it. It cannot be used to travel to most foreign countries, but the bar code reveals the same information as your passport. You used to be able to use it for entry into Mexico and Canada, but now, who knows? It also used to be valid for cruises to those countries...
