4. This can happen any time premiums are paid via withholding... Especially, with the add-on dental and vision plans.

As I discovered when I, um, went freelance.

The difference being the entire year’s worth of premiums becomes instantly due the second withholding stops.

Then, if too many people are dropping their plans, they start playing with the dates of the open season. Which if you don’t drop that coverage during an open season then you are auto-reenrolled. In my case, open season had always been two months November through December for years. In a sudden sparsely announced change of terms, it was changed to one month of October. All perfectly legal. Luckily, I caught the change by happenstance. I know several people who didn’t and were responsible for yet another year of stuff they didn’t want or need. But, that’s a different thread.
