I remember a comic strip long ago featuring two anthropomorphic turtles who were college students. (I searched but couldn't find it on the Internet.) One turtle said that he wasn't sure what he wanted to major in and the other turtle told him about a test he could take to help him decide. He was interested, but he then asked where he might find a cheat-sheet for it.
The joke was that there would be no point in cheating on a test that's intended to help you learn something about yourself.
The cognitive test that Trump took had the same purpose. It's not a test meant to prove something to others, it's meant to tell you something about yourself, therefore it neither has nor needs any safeguard against cheating. I strongly suspect that Trump knew exactly what test he would be taking and he simply memorized the answers, and the doctor who administered it to him played along.
