Whenever we had to do a "lessons learned", it was for one purpose only... To cover our asses and make it seem like we were doing a really good job until something no one could possibly have foreseen occurred. Now however, we have studied it closely and it will never happen again.
It's a big old piece of bullshit so everyone at the top can pat themselves on the back, take no responsibility and pretend they all smell like roses. It is usually followed by a very quiet fucking of the little guys that sounded a warning well before the issue arose and were prevented from doing anything by those in charge so a few bucks could be saved.
