1. Beats me.
In Astonishing X-men, she's trapped in the Breakworld bullet, but I don't know what connection that title has to X-men canon - it seems only loosely tied in with the rest of the X-titles. I guess Whedon expressed a desire to keep the title out of any crossover events, but at the end of his run it's supposed to reveal where it fits.

There's nothing in preceding X-titles to Uncanny #496 to say what Colossus is talking about at the cemetery.
