if you leave it on too long, or make too strong a solution. Been there, done that ;(
On another note - and I learned this living in a place with horrible, horrible water - try to remember to turn all your dark clothes (black and dark blue) inside-out before you toss them in the wash. Helps hold the color (and can save you from some stain issues, also). Apple cider vinegar helps hold the color in darks, too.
