Because the camera angle is straight down on it, but it's a piece of looped string or cloth sticking up from the edge of the sleeve, but it's straight into the camera so it blends into the seem...

I see what you're seeing, but it seems like it would have to be big, and exactly lined up with the sleeve/seam...

It doesn't have to be that big..It's actually not exactly lined up, you'll notice the "bulge" of darkness at the point where the shadow meets the sleeve that I suspect is the loop...and the angle of the sunlight elongates the shadow...

You can tell from the shadowing on the rest of his body...The right side of his face is lit by the sun and the other side is in shadow...

Edit - looks like the shadow from a plastic D ring, which is sticking up in line with the edge of the sleeve.
