what should I do with it? the 2005 Ask america Survey

My late husband recieved this in the mail today ASK AMERICA 2005 NATIONWIDE POLICY SURVEY . Seems he was selected along with 999 others in our State to tell congress what he thinks.

I'm sure he doesn't think much anymore.. since he's dead, but I don't know if I should fill it out in his name, my name or just toss it in the can?

I found out shortly before he died that he was a registered republican, needless to say I was shocked as I thought we agreed on most political issues. But thats neither here or there now.. what do you all think I should do with the survey?
