Second in THREE DAYS, and the fourth one since 15 people were killed in an explosion in Texas City in March.
No one hurt in this one, which was at another facility (not the gasoline refinery) but... what the hell?

a collapse of safety regulations. Happened under Reagan with the airlines. Reagan fired the air traffic controllers and loosened regulations, and for the next few years planes were falling out of the sky from lack of inspections and overworked controllers trying to land planes in storms.
In Texas, corporations regulate themselves, and whistleblowers are fired. A fish rots from the head.
