watching "The Steve Allen Show" and he was doing some stunt and he slipped and said "OH HELL!" The sirens went off, the spotlights went into search mode and there was general shock and guffawing around the studio. Allen clapped his hand to his mouth. I'm sure the 'cough' button was intro'd into telly studios soon after...I don't know-it might have been '62, but it sure as hell wasn't planned!
