It's better that way, you can follow what few inter-episode arcs they litter the show with.
Imagine a Doctor as the world's best auto mechanic. He's presented with a problem at the beginning of every show that seems unlikely to ever be solved, and then he solves it.
He also does it by being contrary and surly to the sick people. Just like an auto mechanic.
No offense to the auto mechanics who are great, but you guys are more rare than diamonds.
