A presoak in hot water with an oxygen bleach (oxiclean or something similar) and simple green will get any stains out, even old ones.

Like Cascade. The powder, not the liquid or gel or tabs or anything else. Powder.
Hot water. Bleach. Dishwasher detergent. Borax. Soak for a while and then launder.

the picky resale shops that take kids clothes recommend the dishwashing detergent trick and it works.
