Fortunately, I've outgrown the desire to wear fashionable shoes, since it's hard to find Eff Me shoes for Neanderthal feet. It's not easy feeling sexy when your feet hurt.
I loves me my Birkenstocks (size 39)!

I have to wear either sneakers or soft Italian shoes. A salesman once told me that the Italian makers (Bruno Magli, Ferragamo) send all their narrow shoes to the United States.
I wear an 8 triple-A or sometimes a Quadruple-A (narrowest they make).
I'm five foot three and small boned. My dad wore a 13 Triple-A, wore Ballys.
My daughter wears a 7 triple-A, I think. Her dad has long skinny feet too.
I like shoes that are flat and pointy.

I can't imagine trying to find shoes that are AAAA all the way up.
The best fitting thing I've had on my foot recently is an ankle brace -- who knew they made them in 9.5 AA?

I have those very narrow heels too. I would love to find shoes that don't rub at the heel, even sneakers, even when I lace them that special "narrow heel" way, even with heel pads.

Narrow shoes are getting harder and harder to find. I find that even shoes that come in narrow aren't necessarily carried in stores. I found a pair of Donald Pliner shoes a few years ago at an upscale shoe shop, but they did not have then in narrow, said that I couldn't special order them either...they only came in B width. A couple of years later I found them in AA at zappos.com. This store is so big on personal service (will phone me if they locate a certain style & color for me) but they could not manage anything as basic as my SIZE.
Those that carry widths other than medium now carry Wide but not narrow. Dress shoes are hell to find because the heel rarely fits even if I am lucky enough to find a narrow width.
When I was a kid I used to be able to select AA or AAA widths and have a choice of AA AAA or AAAA heels. Soon they will manage the same miracle they did with lots of clothes and shoes will come in S M L. And they will all be flip flops.
www.zappos.com is a good resource for narrow shoes. Sudini shoes I have worn in the past and they tend to come nice and narrow although I have not bought a pair from them for a few years but I thought the leather was of excellent quality and the shoes really nice and slim.

You may already know of it but it is a godsend for me...www.zappos.com. The search function allows you to select out the size and width and the style, heel height so it is pretty easy to zero in on what's available. Plus it's free shipping and free to return them (within 365 days!) if they don't fit. I'd be barefoot without them.

actually, 16-17, depending on the brand. i don't think that i've ever encountered shoes in 16 1/2- they don't generally seem to mess with half-sizes at that end of the spectrum. i know that oddballshoes.com doesn't stock 16.5. i can wear vans size 16 w/out socks- but for nike's as well as most dress shoes, work or hiking boots- i need a 17.

I hate having to wear size 11 or above; they get to frigging long and I've to stuff 'em.
Otherwise the shoes fall apart in 3 months because of either my feet being too wide or because they're cheaply made shoes. Actually, the Dr Scholl shoes I've had have lasted 6 months and haven't fallen apart yet.

Often these tend to be the smallest size they stock, so I'm glad my feet aren't just a tad smaller as buying shoes would be much more complicated.
I've noticed that my size does tend to be the display size at some places. I've heard you can get a discount on those, but I've never tried...
