Of course Obama referred to McCain's policies, but on Letterman, when asked, Obama said that if he had wanted to refer to Palin with the phrase, she'd have been the lipstick and McCain would be the pig!!!

The whole McCain mess, person and politics, was a mess, a pig, and they tried to put lipstick on it (Palin).

Seriously, if he straight up said "Sarah Palin is a pig" that is not sexist. It is an insult but not a sexist insult, it has the same meaning regardless of the sex of the target.
