Freedom of Information Act;

I am trying to get my files from the DOJ. After spending a 1/2 hour at their site, I may have the info for doing this. I printed out DOJ-361 and now I believe I need to send what they are calling a 'request letter.' If anyone has any knowledge on this, I would appreciate a heads up. Thanks in advance!

*Edited to include the fact that I'm looking for any record that I may have been spied upon from the early 70's until now. I guess now's a good a time as any to indulge my paranoia
