$38 Million Computer system down for a month in Iraq & nobody noticed
Associated Press - WASHINGTON (AP) - A troubling sign of mismanagement as Iraq tries to rebuild its government.

A report says a computerized Iraqi accounting system was down for a month and no one noticed that it was not working. The U.S. spent at least $38 million on the system.

The report from the special inspector general for Iraq reconstruction details slow progress on some projects and waste and chaos in the management of another.

The inspector general has recommended that no more money be spent on the system until someone figures out exactly what Iraq's ministries can use and sets out plans for developing such a program.

1. they used an arabic accounting system that did not translate into
the US system. Nor did the US try to educate, train, or even suit the needs of the Iraqis.
I do believe that there is a term for this snafu.
