I don't think I'd ever call a stranger a "bitch," but whenever I've used the word, it's always been an affectionate phrase to a woman who had done something I thought of as bold and not "nice" in terms of societal expectations. Always a compliment, and always taken that way. Though I do remember shocking my mother once calling my then-spouse a "bitch." She was twice as shocked when Spouse smiled at me for it.
There's only word I'll never use under any circumstance--the N word. Won't even quote it. There are two others I'd never use about a human being--one starts with F, one with W. I may say the word in other contexts, but never in the context they are famous for. Every other word to me just depends on the connotation. I'd never call a woman the B word as an insult, but I still use the word in other contexts.
We've all heard perfectly nice words used as the most cutting, dehumanizing slurs, and the worst slurs used as terms of endearment or at least as inside jokes meant to de-power the word. So I don't worry much about what words I can and can't say. If my intentions are wrong, I shouldn't use any word.
On the other hand, if I know a word bothers someone, I'm sure as Hell not going to mock them or flaunt it in front of them, no matter what I think of its use.
