"14 people were working in the containment area of a nuclear power plant in northeast Spain this week when water used to cool a reactor was unexpectedly released.
The incident occurred in the middle of this week for reasons not clarified, not contaminated with any of the employees involved, as the plant owned by Endesa.

Contaminated water flooded the containment building where the workers worked until around the ankles, imbuing the shoe and part of the protective equipment for workers. The nuclear proceeded to evacuate all personnel to flush and decontaminate the building and looked at radiation levels of the affected employees. These did not exceed the maximum levels of radioactivity permitted, he stressed the plant. The water poured radioactivity acquired by being in contact with the reactor, but no particles radiating, as it is constantly filtered by chemical, plant sources said they considered "moderate to low" levels of radiation fluid poured into the area.........................."

Nuclear adviser to Japanese gov’t quits, holding back tears — They “have ignored the laws”

"A special advisor to the Japanese government on radiation safety resigned Friday, saying that he was dissatisfied with the handling of the ongoing crisis at the Fukushima Daiichi nuclear power plant.

Toshiso Kosako, a professor at the prestigious University of Tokyo, said at a news conference that the prime minister’s office and agencies within the government “have ignored the laws and have only dealt with the problem at the moment.” Holding back tears, he said this approach would only prolong the crisis. <...>

Mr. Kosako was appointed on March 16. In announcing the appointment, chief government spokesman Yukio Edano described him as someone who “possesses outstanding insight and expertise in the field of radiation safety.” <...>........"

"Data released on April 28, 2011 by TEPCO is now unequivocal in showing ongoing criticalities at Unit 2, with a peak on April 13. TEPCO graphs of radioactivity-versus-time in water under each of the six reactors show an ongoing nuclear chain reaction creating high levels of “fresh” I-131 in Unit 2 <...>

When a nuclear reactor goes “critical” it means that the fissioning of U-235 or Pu-239 becomes a self-sustaining process, called a chain reaction <...>

But instead of seeing that expected decrease in I-131 levels relative to Cs-134 and Cs-137 in the regular TEPCO press releases, I-131 was seen to be increasing, instead of decreasing as the physics said it should. <...>

“Outlier” Unit 2 has I-131 levels roughly 20 times its levels of Cs-134/137. The only possible source of I-131 would be “pockets” of molten core in the Unit 2 RPV settled in such a way that the boron in the injected water is insufficient to stop the localized criticalities......."

Tepco Slows Water Injection at Fukushima Reactor to Curb Risk of Explosion (Unit 1)

"Tokyo Electric Power Co. slowed the pace of water injection into one of the six reactors at its crippled Fukushima Dai-Ichi nuclear station after a drop in pressure and temperature increased the risk of an explosion.

“We wouldn’t want the pressure of the containment vessel to fall below that of the atmosphere,” Junichi Matsumoto, a general manager at the company known as Tepco, said at a media briefing in Tokyo today. The water flow was reduced to 6 cubic meters an hour today from 10 cubic meters, and nitrogen is also being injected to help avoid a hydrogen explosion, he said.
