7. Well, it could have been worse.
Several years ago, I had a cold, and because my regular doctor was on vacation, I was seen by another doctor in the office, whom I had never met. He came into the examination room, and asked me how I was feeling. After I told him I had a bad cold he asked about my chest. When I told him it was fine, he asked if the incision was giving me any trouble. The following conversation ensued:

Me: "What incision?"

Him: "Don't you have a T incision on your chest?"

Me: "No."

Him: "Didn't you have heart bypass surgery last month?"

Me: "No!"

Him: "Are you sure?"

Me: "Yeah, I think I'd remember that."

Right name, wrong chart, and a doctor who didn't want to believe his staff had made an error.
