He HAS put on quite a few pounds around the middle, since his knee injuries last year. The poor man might be in denial about this, as he tries to squeeze into his old pants etc. (one reason why his fly apparently came unzipped at the APEC meeting?). Maybe his staff will have to replace them with the next largest size, without telling him.

Alas, I fear that it will become more obvious as the summer approaches and he goes without his coat and suit jacket.
