I was a registered nurse, there is a type of toilet which has on top of the tank area a small sink with a faucet. The water for both comes through the same pipe, but the water is not recycled from the toilet outlet to the faucet.
Whether it is to save money, degrade incarcerated people, or both, I don't know. It isn't really necessarily unsanitary though, and I've never seen incarcerated people soil the faucet/sink.
