Filing for SSDI...help!
I want to apply for SSDI for my ADHD/ODD/Aspie son. I was going to get the forms from the SSA website, but there are several relating to children, and I don't know which ones I need, or if I'll need them all. Does anyone know what forms need to be filled out? I want to get them all done and in at once if I can.

1. Print them out, and mail them in. Don't do the electronic!! I tried
to file in November online, it took over five hours to complete the forms, then when I called to double check four weeks later, they told me they had no record. :banghead:

When I had the phone appointment with my local office, the woman was able to pull up the forms I had filled out, in about 30 seconds... the ones the Federal number "couldn't find".

2. That's what I'm trying to do
The problem is, I can't seem to find a list of forms needed to apply for SSDI for a kiddo. I did a search, and got a list of forms, but I don't know if I need them all, or if there are others.

I'll check out the other group...Thanks!

and don't leave until you've answered every question you have

don't guess which form to send in, that's called gambling

Edit: I had two phone appointments before I made an appointment to come in to the office. They cancelled the first appointment and the second one called two hours late and I was busy.

they usually can speed the process up, and seeing as it's a child, they should be more than willing to help. (I had someone who worked with the agency who determines wether the disability is real or not, tell me that).

There are two ways to start the SSI disability process for your child. First, you can go to your local Social Security office and with the assistance of a local office representative, fill out the SSI application packet. Staff there will interview you and send you forms to sign and take into your local Social Security office to complete the filing process.

In either case, you will be interviewed about your child’s medical condition, functional limitations, and treatment history. This information will be used to decide whether your child satisfies the SSI childhood disability standard.
