Premarin topical cream
I want to find a way to put this without asking for medical advice. I am not asking for a diagnosis or any such thing.

When I started menopause, I took premarin orally for about six months. I worried about the things I had read about that drug. During that time, I dieted and exercised, because I had gained some weight as an initial side effect of menopause. I also went back to school. I decided that making positive changes in my life was the best way to go through menopause. It certainly beat using a drug that made me uneasy.

Now I have Grave's Disease. Perhaps related to this condition, or perhaps not, I have some problems with urinary urgency and occasional incontinence. I have what is called a rectal seal. I have been seeing a urologist, and taking a drug that I think is neither helping nor harming me. I asked the urologist for a referral to a gynecologist for treatment. I had understood that the gynecologist would do a repair or give me a pessary. He prescribed topical premarin.

I was put off by his attitude. He dismissed my concerns about this drug. He seemed to think that the biggest deal of all for me was the fact that intercourse is painful and currently out of the question for me. Actually, I would like FIRST of all to go out without mapping all the bathrooms on my route. I would like to be able to sit through a meeting or an event without going to the bathroom two or three times. I would like to get rid of the pain often associated with this, and not have to worry about having an accident. Those things come first.

I understand that this drug can help some people. But I am worried about it, in light of the research that has been done on premarin, the fact that I am hypothyroid due to having my thyroid destroyed with radioactive iodine, and the variety of other drugs I am required to take. This doctor did not seem to care about any of that. I wonder if he even read my list of meds. I did have to remind him that I am under treatment for another condition. I did tell him that I had expected another treatment option, not another prescription drug.

I am seeing him in another month. I will take the drug and try to keep an open mind. But it will not be easy.

1. Dear murielm,
please find another doctor or doctors, asap. Any internists in your area?

I have no specific advice about your maladies, but seems to me that whatever this joker does for you won't be satisfying.

2. Maybe I am not being open-minded.
I see so many doctors these days that I am starting to hate all of them.

I will try this course of treatment.

4. Fire him
Second, educate yourself about what's going on. What you have is spelled "rectocele," and a good overview is at http://www.med.umich.edu/1libr/aha/umrectocele.htm

Your incontinence should be evaluated by a urologist. Surgery is indicated for some women, drugs for others, and still others can help themselves by doing Kegel exercises, trying to stop the urine stream 5 times every time you pee. http://en.wikipedia.org/wiki/Kegel_exercise A good overview of urinary incontinence is at http://www.niapublications.org/agepages/urinary.asp It can generally be controlled and often cured.

Given the state of recent research into HRT, physicians who don't listen to women who are opposed to taking it need to be educated on why they are so opposed. If you can't educate him, fire him.

He is correct, though, that the thyroid medication doesn't contraindicate HRT.

5. Thank you warpy!
No one ever bothered to write it down for me or explain it. I have been seeing a urologist. After a couple of years on the worthless medication, I asked for this referral.

I am glad to know the correct spelling. I am grateful to you for giving me some direction so I can make a decision about this.
